Required Documents for Providers
If you are a healthcare provider and would like to schedule a sleep study, we ask that you please provide the following:
- Patient Demographics Sheet
- Insurance Information
- Latest Office Notes or History and Physical
- Certificate of Medical Necessity (CMN), signed by Physician Download
We will verify the patient's insurance, and complete pre-certification if needed. We are committed to keeping you informed throughout the entire process.
